The Ohio State Highway Patrol is investigating a three-vehicle crash in Defiance County that resulted in one fatality Thursday evening.
According to a news release from the patrol, the crash occurred just after 9 p.m. on Domersville Road near Flory Road in Adams Township.
Preliminary findings indicate that a teenage driver was traveling north on Domersville Road in a Chevrolet Malibu when the vehicle went left of center and collided with a minivan. The impact disabled both vehicles. Authorities said debris from the crash also struck a nearby pickup truck, which was able to stop safely.
The driver of the minivan, identified as 47-year-old Jennifer Bartlett of Alvordton, Ohio, was pronounced deceased at the scene, according to investigators.
A passenger in the minivan was transported to a local hospital with serious injuries. Authorities reported that neither the driver nor the passenger in the minivan was wearing a seatbelt at the time of the crash.
The teenage driver of the Malibu, who was also not wearing a seatbelt, was initially taken to a hospital in Defiance for treatment. Due to the severity of injuries, the driver was later airlifted to a hospital in Columbus for further care, officials said.
The driver of the pickup truck was not injured.

Multiple agencies assisted at the scene, including the Defiance County Sheriff’s Office, Adams Township Fire Department, Defiance City EMS, South Richland EMS, ProMedica Air, and John’s Towing.
